By scheduling online, you agree to the policies below ​​​

 

We require a $60 non-refundable deposit to reserve a New Patient Appointment online.

This is done to reduce the frequency of No-Show-No-Calls.

(Online New Patient Scheduling may be limited during the holidays) 

 

Scheduling Policy

Please be courteous to fellow patients and give us 48 hours' notice if you need to reschedule.

No-Show-No-Calls takes away the opportunity for fellow patients to be seen; this is especially true for first-time visits because of the extended length of the appointment.

If you do not give us at least 24 hours' notice to reschedule or notice to cancel your appointment, or if you do not show up for a scheduled appointment, you will be charged a late rescheduling fee of $20 and/or the total amount of your appointment based on the treatment plan for that visit and may be asked to prepay in full for your next appointment.

THIS INCLUDES NEW PATIENT APPOINTMENTS

A patient who is a no-show three times or more may not be rescheduled for future appointments and may be dismissed from the practice.

 

Late Arrival Policy 

​

If a patient is more than 15 minutes late to their appointment, the appointment will be canceled, and the patient will be charged a late cancellation/rescheduling fee.
