By scheduling online you agree to the policies below
All new patients are required to schedule a new patient appointment
The New Patient Appointment is an hour long and consists of a problem-focused physical, review of medical history, which may be followed by treatment. Most patients can be treated the same day however sometimes further testing may be required prior to treatment. We will discuss a treatment plan based on your presentation and symptoms during this appointment.
Please be courteous to fellow patients and give us 48-hours notice if you need to reschedule.
No-Show-No-Calls take away the opportunity for fellow patients to be seen, this is especially true for first-time visits because of the extended length of the appointment.
If you do not give us at least 24 hours notice to reschedule or notice to cancel your appointment, or if you do not show up for a scheduled appointment, you will be charged a late rescheduling fee of $20 and or the full amount of your appointment based on the treatment plan for that visit and may be asked to prepay in full for your next appointment.
THIS INCLUDES NEW PATIENT APPOINTMENTS
A patient who is a no-show three times or more may not be rescheduled for future appointments and may be dismissed from the practice.
Late Arrival Policy
If a patient is more than 15 minutes late to their appointment, the appointment will be canceled and the patient will be charged a late cancelation/rescheduling fee.
We are unable to make accommodations at this time for late arrivals due to Covid-19 mitigation
and will be strict with our scheduling policies
If you have any questions regarding these policies, please let our staff know and we will be glad to speak with you in more detail.
