The overall health and well-being of our patients is our number 1 priority and is the reason why we go to great lengths to make routine wellness care and active treatment of injuries/illnesses convenient and affordable for all.
Since recommendations will vary from person to person and as your needs may change as you progress through treatment we charge per session by services provided. We discuss what makes sense for you given your condition each visit and what to expect in the form of a treatment plan.

Bounced Checks will result in a
$20 fee and
possible prosecution under the Indiana Code Title 35. Criminal Law and Procedure § 35-43-5-5.

If You Have Health Insurance Coverage
StoneWater Acupuncture & Chiropractic will provide what the insurance industry calls a "Super-bill" that can be submitted directly by patients to their insurance companies for reimbursement of covered services.
StoneWater Acupuncture & Chiropractic does not guarantee claims will be reimbursed by insurers. Insurance companies most often will only pay for treatment that is considered medically necessary thus elective maintenance care and wellness care will most often not be covered.
What is Active Care? What is Elective Maintenance and Wellness Care?
Active Care is defined as treatment rendered to address a health concern that is impacting your ability to perform activities of daily living and during the course of treatment, you demonstrate continual improvement. If you reach a point in treatment where improvement is not demonstrated by improved function then insurance companies may deem any care rendered after as elective maintenance or wellness care.
If you regress due to an event, have a change in symptoms, or new condition then active treatment may resume.
StoneWater is In-network and Does Accept Traditional Medicare as a Participating Member.
Traditional Medicare only covers chiropractic manipulations/adjustments during an active treatment plan. Other services performed during treatment may be covered under a supplemental plan. Traditional Medicare and most supplemental plans do not cover wellness visits or maintenance care these types of care are considered elective. You must present both cards and ID at the time of your appointment. StoneWater is not In-network with Medicare Advantage Plans.
*Currently, Medicare only pays for Acupuncture for the treatment of low back pain.* Acupuncture is new for Medicare and there have been some bumps in the road with this coverage and legislators are still working out the details of this coverage. Currently, because we do not have a supervising MD on staff we can not bill for Acupuncture treatment under Medicare or Supplemental Plans.
Workman's Compensation, Automotive Medical Claims, and Private Insurance Policies
We will supply superbills and or 1500 forms just as we would with those that will submit claims to their health insurance plans these can be submitted to your lawyer and insurance adjuster for re-reimbursement for expenses relating to your injuries. We advise all patients to be familiar with their insurance plan and coverage as we can not guarantee that the plan will cover all or any care provided.
